- To assist the patient in searching for meaning in personal life experiences surrounding the patient’s illness or disability, as appropriate.
- Encourage the patient to collaborate actively with home health care staff members and other health care providers regarding the provision of home health services.
- Assist the patient to make choices and decisions about home health care services relative to the patient’s self-defined unique needs.
- Serve as a resource and provide health care education for the patient, family and the community.
- Assure a high quality of personnel through efficient and effective initial screening, periodic evaluation, and an organized program of in-service education for all personnel.
